massive glossary expansion

This wider format should allow the glossary to sit at the front of books, and introduce the key concepts.  Each book should stand alone, to a degree, so the glossary can help bridge that gap.

The wider glossary should also act as a primer for the rest of the book - it will be the first thing people read as they open the book.
