Add yourself to a block list

Add a simple feature where a user can unsubscribe themselves from all lists and block themselves from ever being added to any lists in the future.

• User fills out form or sends mail to an address.
• User clicks or replies to mailback verification.

Done, then the user is unsubscribed and blocked.
