fix missing items/tidy Table of Contents

This isn't a content update, it's a meta update (updating the guide's source's pointer links).

  • fix missing Table of Contents items
  • tidy up
  • some things could be shortened to be easier to reference
  • remove stale items that weren't referenced in foot of document
  • remove in-sentence : where it shouldn't be there
  • pick between 10/11 or 10 and 11 (leaning towards 10/11 which sounds better)
